Zellweger syndrome

Zellweger syndrome is the most severe peroxisome biogenesis disorder, characterised by absent or non-functional peroxisomes, profound hypotonia, seizures, craniofacial dysmorphism, liver disease and renal cysts.

It is an autosomal recessive disorder caused by biallelic pathogenic variants in PEX genes, and most affected infants die before their first birthday despite intensive supportive care.

The syndrome was first described in 1964 and remains the reference disorder for the Zellweger spectrum, which also includes neonatal adrenoleukodystrophy and infantile Refsum disease.
